Abstract

The article deals with the questions of assessing the dental status of patients and the influence of intraoral piercing on the hygienic indexes of the oral cavity and the electro kinetic mobility of nuclei of buccal epithelium cells. Material and methods. The research was conducted on the basis of the Department of Therapeutic Dentistry of the Kharkiv National Medical University in 2012-2016. A total of 84 patients with orphasic piercing were examined. Patients were divided into 3 subgroups, depending on the material of the decoration. The 1st subgroup comprised jewelry made of medical steel (n=52), the 2nd subgroups involved titanium jewelry (n=21), and the 3rd subgroup (n=11) included decorations made of polypropylene. It was noted that the length of wearing jewelry was from 1 to 4 years. The control group consisted of 15 clinically healthy patients in a prophylactic examination (n=15). Patients were questioned and oral, clinical examination of the oral cavity with the definition of the hygiene index (OHI-S) Green-Vermillion, the papillary-marginal-alveolar index, as well as the determination of the electrokinetic mobility of the nucleus of the buccal epithelium cells (ECRI CBE) by V. G. Shahbazov's method. Results and discussion. During the examination of patients in the main group, it was found out that the puncture of the tongue occurred in 45 people, the puncture of the upper lip had 17 people, the lower lip was punctured in 20 cases, and the bridges of the upper lip were punctured in 2 people. Ornament from various materials was determined in the cavity of the mouth. Medical steel was found most often, it was in 52 people (61.90 %). Titanium and polypropylene jewelry were detected in 21 (25 %) and 11 (13.09%) people, respectively. Patients for piercing of the mouth used oral piercing of one type of material. The survey found that complications in healing in patients with medical decorations were determined in 28.88 % of cases. The jewelry of titanium and polypropylene were met in 4.76 % and 9.09 % of cases, respectively. The allergic reactions to medical steel were noted in 3.92% of the people in the main group. Cases of allergy to other materials were not detected. It was found that the OHI-S hygiene index in the 1st subgroup of patients who were implanted with medical steel was the highest and elevated compared to the control group, which indicated the greatest impact on the hygienic condition of the oral cavity in patients with piercing. The papillary-marginal-alveolar index was elevated in all three subgroups compared with the control, which indicated inflammatory changes in the alveolar mucosa at the site of contact with the ornament. Changes in the index of electro kinetic mobility of buccal epithelial cell nuclei in patients of the 2nd and 3rd subgroups were the results of mechanical effect on the oral mucosa of titanium and polypropylene jewelry. Conclusions. Thus, the most significant deviations of the index of electro kinetic mobility of the nuclei of buccal epithelium cells were observed in patients of the 1st subgroup with jewelry made of medical steel, which characterized the development of stable membrane pathology during prolonged stimulation with Ni-capacious material.
