Clinical and "Clip-On" Thinking of Doctors-Iterns

Abstract

Clinical thinking is a professional, creative solution of diagnostic issues, treatment and definition of disease prognosis of the patient. Lack of clinical thinking generates in the medical environment such phenomenon as "paramedicism ", the basis of which is modern concept – "clip-on" thinking, that is formed as a specific response to the growth of information. Materials and methods. Clinical and "clip-on" thinking of doctors-interns different specialties (medical profession and dentists) were studied by us. "Clip-on" thinking was diagnosed in 21,1% of respondents, trained on a budgetary basis and 37,8% of respondents on a contract basis. Higher percentage of risk group and lower "long-term" thinking was diagnosed in doctors-interns on a contract basis, than on budgetary group. Results and discussion. The average overall result, the percentage of "clip-on" thinking and the risk group of interns on a budgetary basis, respectively, were 9,8%, 44,2% and 25,1% less than interns on a contract basis. At the same time, the percentage of people with "long-term" thinking of interns different specialties in the medical profession was more than twice as high as this compared with interns-dentists. Conclusions. "Clip" thinking pushes aside clinical thinking, the principles of a medical deontology. "Clip thinking" is an irreversible process and to fight against is useless and dangerous. Modern medical postdegree education demands formation of qualitatively new approach to educational process, based on formation and development of clinical thinking taking into account psychological features of modern youth. This phenomenon demands detailed social, andragogical and medical research, creation of new educational technologies based on "live" communication. "Clip thinking" possesses not only shortcomings. It is just development of one cognitive skill at the expense of others. Now it is still like "the child in a cradle", and the results of this growth depends on us.
